That was self-insured. One of my co-worker's husband was on the list for a liver transplant. Blue Cross, the plan administrators (actually the hospital administration), kept denying the transplant and related procedures. She knew what was going on; the hospital (oh so compassionate! We care about our patients!) was trying to run out the clock hoping he'd die before he could get the transplant. He didn't and eventually got the transplant, I believe attorneys were involved.
