But I think that reveals to many Americans that this guy has a screw loose. There's something wrong. Nobody is perfect. But this is off the rails behavior.

Mr Khan is right, he lacks empathy. More broadly, Trump is a sociopath who exhibits behavior characterized by "lacking consideration for the well-being of others, impaired empathy and remorse, and bold, disinhibited, egotistical traits"

A bunch of folks including me are not up to speed on a precise psychological diagnosis but most folks will sense something is wrong with this guy.
