company sent him a lovely letter saying they may not cover all his emergency visits and the operation that took place last week.
WTH?!
He said if they don't he might sue them for fraud or something, saying they took his money under false pretenses.

They are in the business to minimize costs. That is the primary function of an insurance company. If dealing with you suddenly becomes more expensive than paying for the operation, they'll pay for the operation.

that his company routinely denied all higher-priced claims the first time in hopes that the doctor or patient would get discouraged and not resubmit them.
