the hospital/doctor filled out a form which included more information than what you see on a certified certificate now (baby's weight, number of children the mom had to date, parents' occupations, doctor's name etc.). The first certificate that I ever had was just a photostat of that form, with an embossed seal added, since that was the copying technology at the time. (I'm a few years older than Obama.) If I ask for a copy of my bc today, as I have done recently, NYS issues one just like the one Hawaii released for the President.
