nothing serious, but I'm pretty sure I lost in during my recent move. What is the procedure?

If possible get her to write down you # and then you need to contact Health Services to get a new card. sometimes they will ask for ID so they can see you are who you say you are.

It had been stolen and I hadn't got around to replacing it.
I had to arrange with the accounts office at the hospital to be billed privately, and then I was given 6 weeks or so to phone in the new number after the surgery, to cancel the bill I was issued. They gave me the new number at the OHIP counter when I explained that I needed it right away instead of waiting for the card in the mail (because obviously I had procrastinated about going in to get the new one issued). Then I phoned it in, and that was that.
Probably too late for you, but the info may come in handy for somebody else. ;)
