I am using Dreamweaver MX. I know the basics of the software, but I know little about forms.
I want to use forms on pages throughout the site to create an order form. I do not need to be able to accept credit cards, so I am looking to do this without using a shopping cart service. I only want to collect the information, process it, have a response page with the invoice presented to the user and have emails of it sent to the user and myself.
Is there an inexpensive shopping cart program I can use, or can someone refer a book or online tutorial that will help me build this feature on my site using Dreamweaver and scripts?

| 1. What you do will depend upon several decisions... |

Needless to say, you have to decide what to do with the information collected.
Which techniques you use will most likely depend upon which kind of web server you'll be using.
Most webservers on the internet run Linux/Unix/Apache/PHP/Perl, but if you're dedicated to a Windows platform, the best answers to your questions would be totally different.
After that, you have the options of storing the information online in a file to be read later, save the information in an online database, have the information emailed, or a combination of online storage plus email.
So the answers to your questions will be a function of which path you'd want to take.
