There's a very good, very interesting book, "Conspiracy of Fools," that describes the fall of Enron. You might also be interested in the DVD, "The Smartest Guys in the Room."

5. John Clifford Baxter, I believe, was his

full name. I remember this because I have a friend whose husband has the same name, except a different middle name, and he also went by his middle name.
