mistakes are made in stressful combat situations -- how people see what they want to see and/or are trained to see. I frankly have trouble making a blanket condemnation of everyone involved, because it could be seen as ambiguous.

They are simply too distressing for me. The last movie I tried to watch was Platoon in the theatre. I walked out in the middle when Elias (William Defoe) is getting run down by the VA. That's the last war movie/battle scene I've ever watched.
