Bush's behavior and statements most likely fit into delusional disorder diagnosis (at the extreme) with co-morbid anti-social and narcisitic personality disorder features. One has to rule out the remote possibility that his hyper-religiosity may be signaling partial-complex seizures with a temporal lobe focus...maybe fueled by a long history of alcohol or liver damage to the brain.

His cocaine and alcohol substance abuse dependence history and likely premorbid history of learning disorder, only heighten his chances for dementia. Which, if I were a betting man, would be along the frontotemporal dementia vein (maybe Pick's disease). He's got his cardiovascular health working in his favor, so vascular dementia or Alzheimer's disease are not as salient of diagnostic possibilities down the road.
