Tips. Tips! Tips?? Who - when - how much?

1. Pizza delivery - whether they deliver one pizza or three to the same house it's the same labor - does delivering more than one pizza mean a bigger tip?

2. Massage Therapists - they charge an arm and a leg - they call themselves therapists - but I don't know of any OTHER therapist (mental health therapist, physical therapist, marriage therapist) who expects a tip.

3. Self owned business - if a housecleaner running his or her OWN business sets his or her own hourly rate, do you tip in addition?

Believe me, I'm not a bad tipper. If anything I tend to go overboard. But I like to know and understand the expectations.
