2). Mick Ralphs, from Mott The Hoople to Bad Company

3). Peter Green, from John Mayall's Bluesbreakers to the Original Fleetwood Mac

1. Clapton, to a certain degree.
Left the Yardbirds and eventually ended up in Cream.

Jeff Tweedy left Uncle Tupelo (well, they broke up, but still) and formed the much more successful Wilco.

From Temple of the Dog to Pearl Jam and Soundgarden. I think there were others from Temple of the Dog that ended up in Pearl Jam, too, but it is too late to look it up. I still play that CD sometimes.
