A hand-held power driven toothbrush carries a pair of exposed brush
elements and self-contained driving means effective to directly
drive one of said brush elements in oscillation, with linkage
between the brush elements such that the second brush element
is also oscillated, the two brush elements preferably being
oscillated in opposite directions.

United States Patent 5077855

Abstract:
A motor-driven toothbrush comprises a drive unit having a motor and a transmission that simultaneously drive a shaft in rotation and a brush head extension reciprocably in a direction parallel to the axis of the shaft. A brush unit includes a brush head coupled to the extension for lengthwise reciprocating motion on which several bristle holders, each carrying several bristle tufts, are mounted individually for pivotal movement about an axis spaced apart from and parallel to the drive shaft. A crank coupled to the drive shaft imparts pivotal movement to each bristle holder individually via a crank pin for each holder.

Mine has six rocking bristle holders, these sweep back and forth in an alternating fashion, driven by a small crankshaft.

Building the original working prototypes was a hoot.

5. I saw a great antique clamp in a medical set.
like an offset tiny jawed mosquito clamp....didn't buy the surgical set that contained it.

I love clever mechanical things. :thumbsup:
