5. I think he might, out of sheer opportunism...
...and I think if he does, he lights the biggest fire possible in the Clintons.

Hillary still wants to be the first female President. Given that McCain's VP probably has a greater than normal chance of succeeding him, I don't think she wants to be gazumped by a cynical, telegraphed attempt to con female voters into supporting him.

6. McCain does not get to chose...
His VP will be chosen for him. The repub machine is in control now, has been since he got the aledged nomination. McCain is no more, he's gone. The guy up front is just the mouthpiece.
