Just have somebody look at him from the side with a bright light behind him. If the light can be seen shining through his head out his ear, he's not wearing an earpiece.

When we found out about the town-hall meeting with undecideds, we both looked at each other and rolled our eyes. "I want him searched for an ear piece" said I. My husband thought the same thing. We are/were serious.
