6. I think things are going to unfold very quickly.
I'm not sure about the constitutional aspects of naming a new Vice President, but if I can equivocate, I believe she will at minimum be announced as the successor.

12. See 25th amendment
The President nominates a new VP but both houses of congress must approve by a majority vote. So if the switch happens shes in.
