3. What probably happened was when Kelly got the report from the FBI,

he asked Porter about it and Porter said it was all bullshit and a huge misunderstanding, blah blah. And rather than follow up and ask more questions about why the FBI wouldn't let Porter have a security clearance, he believed Porter because of course he didn't do it and even if he did, it wasn't that big a deal, more like a speeding ticket or some other minor violation.

It's all part of that culture of not believing or caring about women.
