would come from the grand jury at Mueller's request. The law is still a little fuzzy about whether a president can be compelled to appear before a grand jury pursuant to a subpoena, but the case of United States v. Nixon suggests he can - although that subpoena was for documents and not testimony. If the subpoena is issued and is challenged (as it certainly would), Trump's lawyers would move the court supervising the grand jury to quash the subpoena. And however that came out, it would go to the D.C. Circuit Court of Appeals and then the Supreme Court. It's also possible it could bypass the Court of Appeals and go directly to the Supreme Court, which is what happened in the Nixon case.
