Why does name calling by Trump sting?

Is it because when Trump talks like a demented juvenile fuck, people in the room laugh and thus validate him.

Or is it because it shocks our sense of common decency that anyone can behave like that?

There's nothing in the law to stop him unless he crosses the slander line. Then he can just plead ignorance to his awareness of his comments being slanderous.

for hitting people where it really hurts the most. I knew kids like that when I was in school and I made sure to stay as far from them as possible, they were bullies and very destructive.

The only chance anyone really gets to verbally retaliate is in a debate or something.
