Since they seem to always be about the same size, and they are sometimes normal, this indicates that it is an intermittent effect applying to both eyes which causes the dilation. That's why he settles on systemic drugs as the probable cause. Injury or topical effects would tend to only affect one or the other eye; disease would tend to always affect them. Systemic drugs affect the whole body, including both eyes, but only when you take them.
