When he asks, "How are you doing? For real?" it just chokes me up! And I'm not usually choked up by political ads.

Without knowing what this was about, my initial thought was about Biden's "How are you doing, for real." question for the nurse in that ad that showed the stark contrast between his empathy and Trump's sociapathy.
But this question in relation to mental health seems to be older.
