He seems energized and interested when talking about himself.

Conversely, he always seems like a sleepy, grumpy nine-year-old reading a book report when he's telepromptering about a non-Trump subject.

That aside, he's always been crazy. Now he's crazy, tired, and beginning to realize that he is defeated.

Dementia, various degenerative cervical spine/nerve diseases, and carpal tunnel syndrome have been mentioned. I could see any of them being at the root of this, including the latter, considering how much of his time is spent typing in tweets on his phone. I'll add another possibility: His muscles are so atrophied from not exercising that he doesn't even have the strength to pick up a glass of water. Also notice that he does a lot more seated "round table" discussions, rather than stand-up press conferences. Is he so out of shape that he can't stand up for more than a few minutes at a time?

Consider that he spends most of his game driving around in a cart. He drives right up on the green so that he doesn't have to walk very far. That does a good job of covering up some of what ails him as far as walking goes, whether it's because he can't, or because he won't. I'm no fan of golf, but even I can see from the videos of him playing that his swing is terrible, and on the weak side. Don't know if that's due to disease/degeneration, or if he always sucked. Probably both.

when trying to do smaller, more precise movements with their hands. It'll just get worse, but it's ok, we're putting his old ass out to pasture come November.

My mom had Alzheimer's/dementia, and my late husband had a stroke. If Trump had a stroke, it was mild. There would be no hiding the effects of a major one. He's more in step with what I saw with my mom. A slow glide path, with an ugly end.

I have big hands for a woman, but my fingers are very short. With all but small diameter glasses and bottles, I have to use two hands to securely hold them - my fingers just don't reach far enough around big containers to get a solid grip so I end up using both hands to hold items. Also, at almost 68 I am losing strength in my grip which used to be able to help with holding some larger containers

He brings up his left hand just as he's trying to rotate the cup toward his mouth. i just had inflammation, but it was extremely painful.
