This runs in my family and I have seen it many times.

Unfiltered thoughts, blurting out inappropriate comments, losing train of thought, rambling stories, paranoia.

It gets progressively worse.

Sucker. Seen first hand mini stroke persons in our family and each time this Sucker is on a Video,boy oh boy,sure appears to have the same symptoms as our rellies. The speech slurring is only one aspect. Watching him walk is the real give away.

If he's in a cycle of using/abusing uppers and downers without regulation and constant testing, he could be having a series of seizures that present as TIAs because of the neurological imbalance that sort of cycle causes.
Speaking as someone who has a loved one on prescribed and maintained psychotropics that has seizures that seem like TIAs if not controlled by the appropriate medication.
