China Deploys Anal Swab Tests To Detect High-Risk Covid-19 Cases

State-run channel CCTV reported that the tests are reserved for high-risk cases, although there does not appear to be a coordinated policy for them, with reports of surprise tests for some individuals..

This includes passengers arriving in Beijing, residents of quarantine centers and, according to local officials, a group of more than 1,000 schoolchildren and teachers believed to be exposed to the virus.

The test involves inserting a cotton-tipped swab about 1-2 inches into the rectum, which will then be tested for the virus.

Not all experts agree with the use of anal swabs to test for a respiratory illness.

The discovery of coronavirus in the bathroom of an unoccupied apartment in Guangzhou, China, suggests the airborne pathogen may have wafted upwards through drain pipes, an echo of a large SARS outbreak in Hong Kong 17 years ago.

Traces of SARS-CoV-2 were detected in February on the sink, faucet and shower handle of a long-vacant apartment, researchers at the Chinese Center for Disease Control and Prevention said in a study published this month in Environment International. The contaminated bathroom was directly above the home of five people confirmed a week earlier to have Covid-19.

The scientists conducted “an on-site tracer simulation experiment” to see whether the virus could be spread through waste pipes via tiny airborne particles that can be created by the force of a toilet flush. They found such particles, called aerosols, in bathrooms 10 and 12 levels above the Covid-19 cases. Two cases were confirmed on each of those floors in early February, raising concern that SARS-CoV-2-laden particles from stool had drifted into their homes via plumbing.

SARS-CoV2 is a novel coronavirus responsible for causing COVID-19, first identified in the city of Wuhan, China and officially declared a pandemic by the World Health Organization. SARS-CoV2 expresses high affinity to human ACE2 receptors, including within the gastrointestinal tract. Patients with COVID-19 exhibit a wide spectrum of GI symptoms including anorexia, nausea, vomiting, abdominal pain, and abnormal liver function tests. Pathogenesis behind gastrointestinal symptoms caused by SARS-CoV2 has been postulated to be multifactorial including disruption of the intestinal mechanical barrier integrity, alteration of the gut microbiome and systemic inflammatory response to the virus. SARS-CoV-2 RNA has also been found in stool samples of infected patients for a significantly longer period than in nasopharyngeal samples, though the implication of this finding is unclear at this time. Liver injury in patients with COVID-19 is usually mild, stemming from immune-mediated damage, drug induced hepatotoxicity, or ischemia from sepsis. Patients with pre-existing liver disease may be at a higher risk for hospitalization and mortality. Given the high degree of infectivity of this disease, healthcare providers will need to remain watchful for resurgence of this virus. Strict protocols should be implemented regarding hand hygiene, isolation, personal protective equipment, and appropriate disposal of waste. It is also imperative to identify patients with gastrointestinal symptoms at an early stage as these patients may have a prolonged course between symptom onset and viral clearance.
