Alternatively, Democrats could negate the automatic cuts if they raise taxes to prevent adding to the debt or abolish the filibuster, another political can of worms.

So if/when it comes to it, could a future gambit be to go to the country and say, well, we have two choices: we either repeal Trump’s tax cuts for the rich and/or introduce a wealth tax on billionaires, OR the Republicans will vote to cut your Medicare?

Those big tax cuts are spending, essentially. Instead of making them pay what they owe, the government is giving them money by cutting their taxes. And tax cuts add to the debt just like spending money on the lower and middle classes does. Most people get some benefit from social programs whether they realize it or not. We need to speak out more on how much spending for schools and healthcare and other things would be cut if we give a big tax break to rich people. I think most people would be against cutting those taxes.

Cedric Richmond explained to Chris Cuomo last night that in this case it doesn't apply because only "permanent" changes passed by reconciliation do that to Medicaid. Apparently the stuff in this reconciliation does not make permanent changes. Or something like that.
