CT doctor's license suspended for giving out blank COVID vaccine exemption forms

A Connecticut doctor’s medical licenses were suspended after the state department of health found she was providing blank, signed COVID-19 vaccine and mask exemption forms to patients.

Connecticut’s Medical Examining Board suspended Sue Mcintosh’s physician and surgeon licenses last week after she was caught giving out signed forms for vaccine, mask and testing exemptions to patients without any sort of exam. Mcintosh, who was retired and is certified by the American Board of Pediatrics, provided the requested forms, already signed, to anyone who sent her a self-addressed stamped envelope.

According to the board, Mcintosh sent back instructions and told people who received the forms they were free to make photocopies and distribute the forms themselves.

The state department of health became aware of Mcintosh’s alleged actions after receiving an anonymous tip in July. A department investigator sent a self-addressed, stamped enveloped earlier in September and received four forms authorizing exemptions for face masks, COVID vaccines, COVID testing and vaccines in general, as well as instructions for filling out the forms.

5. I couldn't see how long it was between her act and her punishment but this is good!

Are companies who check vaccine cards able to verify that the individual actually got the vaccine? Probably no national or state database.

I wish there was a more efficient and timely way for the crazy lawyers to have their license suspended.

I hope that doesn't get me kicked off but what she did was damage to all of us. I use my card for proof of vaccination for engaging with people, DRs visits, the gym etc. Now it won't be worth the paper it's written on.
