Movement conservatives, politics as total war (not good enough to win, the enemy must be destroyed), shrink government until there's no spending to help the Democratic base, take over state legislatures ("We are trying to change the the tones in the state capitols -- and turn them toward bitter nastiness and partisanship" "Bipartisanship is another word for date rape" -- Grover Norquist), corporate funding shifting to Republicans (for example, tobacco industry contributions were evenly divided between parties before 1990 but by 2000 82% went to Republicans), network of organizations and think tanks, media control and all the rest.
It was published in 2004, though, so really old. Wish they'd do an update, if they're still around.
