covid 19 passport

i seemed to have misplaced or lost my covid 19 passport . i need the physical paper one and was given a website called myvccinationrecord.cdph.pa.ca .gov.
i cant make heads or tails of it . i live in california.

i looked on line to find our county health dept phone number and was then overwhelmed . ( mom would say whelmed .) i saw another number and called that . a real person answered it . they had my particulars in their computer system after i provided them some private info and i have a new card waiting for me . i will pick it up next tuseday.i will also get the iphone app and have it on my phone . i also heard that ppl have taken pics with their phones and have it that way. thanks for ur help.
