In our current political environment, I'm more inclined to believe the 'human error' was someone thinking they could tamper/taint the ballots. Anything to prove the electoral system dysfunctional. Even if it was a genuine (egregious) mistake, who the hell is checking the ballots before mailing? With all the distrust spread across the country, the ballots should be squeaky, squeaky clean.

Good for the citizens of this district immediately complaining to the clerk, and hopefully getting a clean ballot replacement.
