West Virginia had a great medical care legacy.
They made it all the way until the 2000s with democrats mostly in care and the UMW as the biggest force. I worked for WVU Hospitals in the 1990s, for a rural state of sedentary Chainsmokers (not all, but many), we had some decent outcomes in some areas, and it showed in the very early months of vaccination.
