My limited understanding of the custom of "we have an arrangement" arrangements is that both partners are usually free to go about their liaisons as long as they do so with discretion (as long as there are no offspring thereof). The public humiliation of cuckoldry is, I think, considered to be beyond the pale.

Melania was pretty much a borderline sex worker who married that ... thing for money, so she can't be imagined to be super high on the "if she has any self-respect" scale, but she has been seen to use her leverage to renegotiate her postnuptial agreement. And she seems savvy enough to have noticed (or at least hired lawyers wise enough to notice) that Trump is 3-for-3 in cheating on his wives, so it's not unreasonable to think that she may have gotten some sort of agreement that enforces penalties for public adultery.
