The changes made to appease Murkowski had to go through the parliamentarian. Some initial proposals were rejected. The ones that were included were approved by the parliamentarian.

Because it only covered Alaska and Hawaii, so the parliamentarian rejected it.
They rewrote the bribe to include, IIRC, 10 states with the most rural hospitals, and the parliamentarian allowed it.
