Medvedev was facing match point. His opponent served a fault. Then, before the second serve, a photographer walked onto a part of the court, to try and get a better angle. They had to stop to remove the photographer. And then, and I haven't seen any justification for this yet (and the umpire couldn't give one at the time), the umpire said "first serve". So the opponent could give a better, riskier serve, knowing he still had a second serve available.
As it happens, Medvedev did win that point. But the umpire seems to have been an asshole, and that can't put you in a good frame of mind for the rest of the match.
