It doesn't look like a footstool to me. It looks like there's a small gilded table beside his chair with a gilded box or whatever on it, perhaps an ornate ketchup dispenser?

The other chairs at the dining table don't have a filigree seat surround, and what looks like the small table's top is similar in finish to what I take to be its foot section, not a footstool.

I was trying to see if the seat of his chair is higher than the others.
Therefore the little footstool.

I use a little footstool when I sit at the table, but that's because I'm only 5'2" and have back issues that are alleviated by using it.
I have to use a cushion at my back because many times my feet don't touch the floor. I know that Trump lies about his height, but I don't think by that much.
