Interesting episode of "Law & Order: SVU."

A witness, a middle-aged man, had once been a big shot lawyer. Then he had a stroke that left him disabled and unable to speak. By the time of the episode, he had long since lost his job, and was homeless and alone. Now why does this seem so plausible, and why does it seem like this is the only "civilized" country in the world where this could happen?
