Folks in my neck of the woods don't have time to recover from necessary surgery, much less elective procedures. Farms and ranches are 24/7 everyday operations, you can't take a day off just for fun and getting time to deal with emergencies requires imposing on friends or relatives who then have to take care of their place and yours.

From the quick look I had of their "farm" I'm thinking her source of revenue comes from somewhere else and she's a "pretend farmer", a few of those around here. Nobody takes them too seriously except to part them from their money when they need to buy feed or have work done on their property.
