Giuliani obviously has a serious problem with alcohol, the overuse of which can have well-known deleterious effects on the brain. Long term alcohol abuse can cause serious damage to the brain's functioning as well as its actual structure. Even without knowing if someone is a long term heavy drinker, the effects can be quite obvious.

and what's even sadder, is that at first, concerned people do go to these alcoholics, and tell them to their faces that they are drinking themselves to death, that they must get help, must stop, etc.
