1. He's seriously unable to see his flaws, 'cause he is, and HAS to be "perfect" in his eyes.

2. Because of this, he does his own hair color/make up, according to Ivanka, when she was asked why his hair color changed in tone from week to week. He doesn't want anyone to see the "before" image.
Of course no one would dare point out the problems with his make up, just as no one bothered to point out that piece of toilet paper.

Which leads me to a 3rd thought....I think people have a hard time "getting" that he is very mentally ill,
because there is a preconceived image of a "crazy" person as flamboyantly wild, yelling, out of control, etc.
while trump tends to have a soft voice, and normal expression in public. He has enough control, still, to have his rages and temper tantrums in private.

And a lot of WHAT he says strikes us as funny/stupid, like when he said "I am the chosen one" or when he stared at the eclipse. "Just a trump silly moment".
Anyone who has ever watched John Hurt's masterful role as Caligula, in I, Claudius, will see how easy it is to for people to not understand the symptoms of insanity until it is much too late.
