2. Also, some insane drivers treat the HOV/carpool lane on the left as the fast lane

It isn't! It's exactly for what the name says: HOVs/carpools.

3. Of course it is stay right except to pass. But that applies to normal traffic, not congested highway

traffic, usually as a result of a wreck up ahead or construction.

I don't know what to think about aggressive drivers (usually in honking big pick-up trucks or SUVs--Coloradoans on I-25 know of which I speak) that try to bully you to the point of forcing you off the road, even though there are zero breaks in the traffic and both lanes are stop-and-go. Sometimes Karma gets them and I've seen them overturn in the median when their attempt to intimidate drivers in front of them while leaving nowhere for themselves to go, backfires.

If someone approaches and maintains a respectable/safe distance, I'll likely move over for them to pass.

... when I know a left exit is coming up, and maybe the guy behind me doesn't know. I don't care, I remain in the left lane to the exit. Doesn't happen often, but we have a few in the Pittsburgh area. Normally I'm happy to stay right and yield to the faster drivers.
