I would send a personal note expressing your love for them, acceptance, and perhaps an acknowledgment that you recognize the depth of the adjustment that will occur for all but that you hope your support is not in doubt.

Also, perhaps a gift card for some clothing or makeup. It doesn't have too expensive, just a little something.
