VA State Bar offers Advance Medical Directive form available online.

I posted this as part of the discussion about Massachusetts' proposed End-of-Life Talks, but thought it might be more useful here, in case anyone needs one.

The Virginia State Bar has a free Advance Directive for Health Care form on their website
that you can download, fill out, have witnessed and distribute to your doctors and family: http://www.vsb.org/site/public/healthcare-decisions-day

You can also register your ADforHC online with the state of Virginia: http://www.virginiaregistry.org/

I'm especially glad a person can register it w/the state so that there won't be any question as to his/her intentions.
