Neither name makes sense, but that what’s we called it growing up in Massachusetts. A dash of Worcestershire sauce adds some dimension.

Simple. Ground beef browned and stirred into good old Kraft Macaroni & Cheese (the fancy kind with the little can of cheese). I still make a low-carb version of this with diced onion added to the ground beef.
