According to Joe, a staffer was assigned to each family member, Melania as the most important was assigned to Manafort himself, and reportedly an assistant worked under him. Even if she did insert the lines herself, whoever was supposed to protect her failed badly. No matter what, Manafort wasn't supposed to go on TV to focus blame on her, but perhaps the assistant or another underling will take the fall.

Then there's the possibility that Trump himself inserted them. He said he was looking over the speech even though she was very capable.
