I really believe that some of us, no matter how hard we work, will not age well. There are others that live a hard life and won't age well. There are some who have great genes and will hold up better than average for many years. And then there are that tiny percent who work their butts off, stay extremely healthy, and have fantastic genes. (Those women are the ones I want to shove pans of brownies in front.)
Cosmetic surgery-a very small amount can make a huge difference, if the surgeon is skilled and the patient does everything in their power to keep up in other ways. (Example: a simple eye tuck or a chin lift can take someone who looks older and make them appear younger.) Personally, I think the problem comes with patients who want too much for very little effort and have expectations that cannot be met. Instead of one or two procedures they want multiple procedures, they want them all at once, and they keep them up over the years. They become almost cartoon-like when they quit being realistic about how they can look.
