The former Soviet leader, famously, was always at pains (as it were) to show the world his health was still reasonably good.

Secretly, of course, he had been dying since around 1977 - and in fact had a near-death experience in 1980. He ultimately passed away in 1982.

"Yes, comrade. Better than ever - thank you! (no U.S. president could ever get away with blowing this much smoke, let me tell you)."
