7. In Missouri

it's $15 per record. It doesn't seem to matter if you pick it up in person or mail in the info.

My birth state of Wisconsin-I had to get a new bc last spring. I ordered it three times and never received it, at $20 each time. (I cancelled each money order after 9 weeks.) Finally, I ordered it via fax with a grand total of $50.00. The total break down, according to my receipt in hand: Receipt of fax-$3., birth certificate-$20.00, expedited service (because all fax orders are automatically expedited)-$20.00, credit card charge-$6.00, misc fee-$1.00. $50.00 total.

I called Madison repeatedly and was informed that this was the best way to guarantee actual delivery out-of-state. All fax orders have an additional charge for receiving the fax, for cc fees. There was no way around it. Oh, and I still don't know what the misc fee was about.
