16. In the seventies and eighties I changed my drivers license between three states.

I started out and ended up in Pennsylvania. Requirements from that time varied from a simple exchange to a written test to a complete driving test. I don't know how it is now, but my original PA test was through my high school drivers' ed class and required no proof beyond my being a legitimate student.
Forms were filled out,of course, but no stringent documentation needed.
