I always figured that the "survey" part was only intended to give the recipient primed and in the mood to make a donation so that the "important things" in the survey could be achieved.

My best cynical guess (although I have no proof) is that someone in the mailroom opens the returned envelopes looking for checks, money orders, and credit card forms... and the surveys are shredded.
