He may just be left with serious injuries.

Or he could have sudden cardiac arrest which will usually kill you, especially if he does drugs that affect his heart rhythm.

But since he's surrounded by people who give a shit about him, he may survive sudden cardiac arrest if he gets immediate medical attention. But that's more likely to leave brain damage.

And then there are strokes.

So many ways for his heart and circulatory system to hurt his huge body mass.
