The obvious alteration in the neck is the added vertical ridge.

The original doesn't show a double chin (although many of those are altered too). Not every photo of people who have double (or triple) chins shows those chins It depends on the angle, the lighting, the posture.

The original picture was published in the Guardian, a reputable newspaper that leans left. https://www.allsides.com/news-source/guardian
