50. I got mine at a mass-vax site sponsored by the local hospital and the CDC.

Nobody asked for insurance information. I'm not sure who ran the clinic you went to, but something stinks there. From what I understand, the federal government paid for these vaccines, and they are not asking the insurance companies to reimburse them.
